Referral Process
If you are a provider looking to refer one of your patients to us, first of all, thank you! Please continue reading for more information on how to refer.
The best way to send a referral is to either fill out the provider referral form above or have the client fill out our “Request an Appointment” form.
We try to schedule 2-3 weeks out depending on availability. Clients should hear from us within 3-5 days of submitting the form.
After the client schedules an appointment with us, they will be asked to fill out an ROI form. We will be in contact with you if need be and are always happy to share our notes and speak on the phone to provide the best service possible for our shared client!
